In general, the parent form appearing in the portal is the preview form defined in Studio for the parent entity. You can modify the preview to add the relevant fields. In the case of a request, the fields come from the Request preview form, not the offering form.

As far as I understand, in the "my approvals" tab, the parent record details forms come from the parent record itself.

So if I work on studio -> Request -> forms -> for example "serviceRequest" form or "full" form, will it have an effect?

you need to select "Preview Request form" in the Request case as advised by Brindusa. Same for Changes.
