There used to be a setting or flag that you could apply to IGA that hid the Access Request menu item. Does anyone know what the setting was and where to set it ?

I agree! It was an attribute that you could use to hide a permission from the catalog. I don't think its in there anymore - I noticed it was missing a few months ago.
The good news is that you could likely achieve similar end-result with the request policies -- with the right policy you can prevent any end-users from seeing the permission to request it.
Is there something specific you are trying to accomplish that you need that attr for?

This just in from a reliable source:
in Configuration -> Advanced look for a key of:
com.netiq.iac.access.request.enabled
and set that from true to false. Save that, and that should do it.

NOTE: The steps outlining using configutil and need to be updated to utilize the IG UI.
